Objectives: to summarize the findings of randomized controlled trials (RCTs) that sought to identify interventions that improve patient compliance with prescribed antipsychotic medication regimes. Search strategy: studies were identified through computerized searches, hand searches of pertinent journals, and references taken from the reference lists of papers already identified in the search. Selection criteria: articles were included in the review if they described original data from an RCT, reporting on the effect of at least one intervention to improve compliance for patients aged 18 and over, receiving antipsychotic medication, and with a diagnosis of psychotic disorder. Data collection and analysis: information on study design features, interventions, and two reviewers, with a third reviewer adjudicating on disputed articles extracted results. The studies were too diverse in definitions, measures of compliance and interventions used to allow meta-analysis. Main results: six studies showed improvement in compliance rates following interventions, although only three of these reached statistical significance. Effective interventions included individualized behaviour tailoring regimes and compliance therapy. Conclusions: medication compliance can be improved by certain, sometimes complex, interventions; however, further efforts are needed in developing effective interventions to assist patients in following prescribed treatment regimes. © 2000 Harcourt Publishers Ltd.
