This review emphasizes that whatever the pathogenesis of pre-eclampsia, the ultimate target is the maternal vascular endothelium which is in abundance within the kidneys. Glomerular capillary endotheliosis is accompanied by decreased renal haemodynamics, disordered renal vascular sensitivity, hypofiltration and loss of permselectivity with non-selective proteinura. Altered tubular function contributes to sodium retention and hypercuricaemia. Occasionally, functional impairment is very severe with both tubular and cortical necrosis. The renal changes of pre-eclampsia are usually fully reversible.
