© 2014 John Wiley & Sons, Ltd. Successful pregnancies have been reported in female liver transplant recipients worldwide. These pregnancies are high-risk, and close collaboration among specialists is warranted. This chapter reviews contraception, pregnancy outcomes, maternal and newborn outcomes, and pregnancy management guidelines for liver transplant recipients. Fertility for female and potency for male liver recipients can return quickly after successful liver transplantation. Prompt counselling and appropriate contraception are key components of post-transplant care in age-appropriate recipients. For women, a variety of safe, effective contraceptives are available to allow planning, given the increased safety and success of pregnancies 12 months after transplant. Fetal mycophenolate exposure is associated with an increase in the risk of malformations, thus delaying post-transplant pregnancy allows for appropriate modification to the recipient's regimen to avoid or minimise fetal exposure. This chapter includes a review of the literature and data from the US National Transplantation Pregnancy Registry.
