Dimorphism with the heat shock protein-70 (HSP70-hom) gene was analysed in 232 donor-recipient pairs of haematopoctic stem cell transplants (HSCT) in relation to the incidence and severity of graft-versus-host disease (GvHD) and generation of graft-versus-host reactivity (GvHR) in an in vitro skin explant model. In addition to the previously reported associations of positive skin explant assay results and the recipient HSP70-hom (-2763) AA homozygous genotype with acute GvHD, the presence of the A allele in the recipient was found to be associated with positive GvHR while the donor A allele appreared as independent risk factor of chronic GvHD. HSP70-hom gene polymorphism associates with the development of post-transplant complications. Recipient and donor genotypes affect the risk of acute and chronic GvHD, respectively.
