Use of COX-2 inhibitors is controversial, as doctors struggle to interpret the trials and balance gastrointestinal safety against concerns of increased cardiovascular risk. There have been improvements in immunological testing for rheumatoid arthritis (RA), with the advent of highly specific anticyclic citrullinated peptide antibodies. This article also looks at novel treatments for ankylosing spondylitis - intravenous pamidronate and antitumour necrosis factor (TNF) therapy. © 2005 The Medicine Publishing Company Ltd.
